Suspension Request Form

If you wish to suspend lessons, you must notify The Little Swim School of your request seventy-two (72) hours’ prior to the commencement of any suspension.
The Little Swim School will not agree to suspend your enrolment if your fees are not paid up to date or you owe The Little Swim School any other money.
If the Little Swim School agrees to suspend your membership you will not be able to attend The Little Swim School for lessons or use any make-ups on your account during your suspension period.
The Little Swim school will suspend your membership for a minimum of two (2) weeks, to a maximum of six (6) weeks.
A Medical Suspension requires a medical certificate to be provided
Your "First Suspended Class Date" is the date of the first class to occur while you are not able to attend The Little Swim School
Your "Last Suspended Class Date" is the date of the last class to occur while you are not able to attend The Little Swim School
Your "Date of Return to Classes" is the date that your child will be back to swimming at The Little Swim School
I, the Guardian, understand that I can suspend my lessons for a fee of $5.00 per person, per week for a maximum of 6 weeks.
